From the State’s Provider Update Newsletter, February 2007:

Progress Reports are to be submitted to the ongoing Service Coordinator at the beginning of months 3, 5, 9 and 11. These will reports will then be forwarded to the Lead ED Team member. This information allows you, as the ongoing provider, to share your observations with other team members and provides a more complete assessment. These reports also ensure that the IFSP outcomes are being addressed and the IFSP team has good information to help determine continued service recommendations.
