Provider Meeting – Important Information

New grant year – time to save money by paying for entire year of meetings

Credentialed Cluster Provider Meetings

This year, the North East Indiana (Cluster B) LPCC will offer 4 Provider Meetings via webcast. These meetings will be scheduled on a Friday, from 12pm – 3pm, at Goshen College in Goshen, Indiana, and will contain 3 hours of credentialing on various topics.

The actual dates of the meetings will change each year due to various scheduling needs, but the dates are usually announced 3 months prior to the next meeting. Please check Provider Calendar for dates to be announced.  The next meeting is May 23, 2008.

Payment Options

There will be three ways to participate or receive credentialing for meetings:

1. Attend the meetings in person at Goshen College in Goshen (South Elkhart County).  You will NOT have to pay for credentialing if you attend live.

2. Attend the meetings live via the web in your home or any other place with web access.

3. Watch the recorded meetings at any time after the meeting is over either via the web or by the DVD recording.

 If you choose to watch the meetings via web (either live or recorded), you will have to pay for the credentialing points. You can choose two options:

a. Pay $50.00 for year. (This saves you $10.00!) This gives you 12 hours of credentialing points and access to meetings either live or recorded. You will have to pay the money one time a year. After we receive proof that you watched each meeting (you just email us the code), we will send you your certificate.

b. Pay $15.00 per meeting. This gives you 3 hours of credentialing points. You can pay at any time before or after a meeting. After we receive payment and proof that you watched the meeting (you can email or mail us the code), then we will send you your certificate.

To take advantage of the $50.00 offering, please send us a check by Friday, May 16, 2008 or you can send us a check for $15.00 at any time before or after the meeting.

All checks need to be made out to: 1^st Kids/Cluster B. Please put in the Memo: Provider Meeting(s) and send to:

PLEASE NOTE: Anyone can watch the meeting without paying the fee! (The provider meeting is to get information to you, so please watch it if you can!) You only need to pay the fee if you want credentialing points.
